Understanding it

Most lower back pain comes from strained muscles and joints, a disc that is irritating a nerve, or a back that has become stiff and weak after months of sitting. It is one of the most common reasons people visit a physiotherapist, and in many cases it improves with the right mix of hands-on treatment and exercise.

Sciatica is pain that travels from the lower back or buttock down the back of the leg, sometimes with tingling or numbness. It usually means a nerve root in the lower back is irritated — often by a bulging (slipped) disc. Physiotherapy aims to calm the nerve down, restore movement and teach your back to handle load again.

What we commonly see

  • Slip disc / disc bulgeBack and leg pain from a disc pressing on a nerve, often worse on bending or sitting.
  • SciaticaBurning or shooting pain, tingling or numbness down one leg.
  • Muscle spasm & strainSudden “catch” in the back after lifting, twisting or sleeping awkwardly.
  • Posture & desk-job painDull ache and stiffness from long sitting, driving or standing.
  • Lumbar spondylosisAge-related stiffness and pain in the lower back joints.
Is it time?

Signs physiotherapy may help

  • Pain that lasts more than a few days or keeps returning
  • Pain or tingling travelling into the buttock or leg
  • Difficulty bending, sitting for long or getting out of bed
  • Morning stiffness in the lower back
  • Back pain after lifting at work or the gym

See a doctor first if you have

  • Loss of bladder or bowel control, or numbness around the groin
  • Weakness in a leg that is getting worse
  • Back pain after a fall or accident
  • Back pain with fever, unexplained weight loss or a history of cancer

If any of these apply, please get medical advice before starting physiotherapy. We are happy to work alongside your doctor.

How we treat it

Your plan, step by step

We ask how the pain started, check posture, movement, nerve signs and strength, and look at any X-ray or MRI reports you bring.

  2. 2

    Pain relief

Manual therapy, soft-tissue release and, where suitable, modalities or dry needling to ease spasm and pain.

  3. 3

    Movement & core work

Graded exercises for the spine, hips and core so your back can handle daily load again.

  4. 4

    Home plan & prevention

Simple exercises, sitting and lifting tips, and a plan to reduce the chance of flare-ups.

Questions

Good to know

Can physiotherapy help a slip disc without surgery?

Many people with a disc bulge improve with physiotherapy and time, and surgery is usually considered only in specific situations. After assessing you we will tell you honestly whether physiotherapy is suitable or whether you should see a spine specialist first.

Should I bring my MRI or X-ray?

Yes, please bring any reports and prescriptions. They help us plan treatment. You do not need an MRI to start physiotherapy for most back pain.

Is bed rest good for back pain?

Long bed rest is generally not recommended for most back pain. Staying gently active, within comfort, usually helps recovery. We will show you which movements are safe for you.

How many sessions will I need?

It depends on the cause and how long you have had the pain. After the first assessment we explain the plan and an expected number of sessions, and review progress as we go.
